OK, the problem is stranger than I imagined.

After rebooting my dom0 (the mailing list runs in a Xen guest),
crond stopped running cronjobs in the Xen guest.  Once I restarted
crond (and exim, out of paranoia) logrotate started running again
and the mail queue started being pushed out.

I have no idea yet what causes crond to stop running after a dom0
reboot (and guest save/restore), but will be looking into it...
